- Athletics Participation Clearance (PDF - 18 KB) - Please complete the Athletics Participation Clearance form for each athletic program and return to the school as required by the Mississippi High School Activities Association.

- Medical or Dental Prior Approval Form (PDF - 19 KB) - According to Mississippi Code Section 37-13-91(e), all medical and dental appointments must be preapproved for the absence to be an excused absence. The only exception is in the case of an emergency.
