Athletics Participation Clearance (PDF - 18 KB) - Please complete the Athletics Participation Clearance form for each athletic program and return to the school as required by the Mississippi High School Activities Association.

Medical or Dental Prior Approval Form (PDF - 19 KB) - According to Mississippi Code Section 37-13-91(e), all medical and dental appointments must be preapproved for the absence to be an excused absence. The only exception is in the case of an emergency.
Request for Release (PDF - 14 KB) - This form needs to be sent annually to Pontotoc County School District for School Board approval to release students from the district.
